I'm in the process of applying for an affix.   If I phone the kennel club, will they be able to check whether the name I have chosen is ok or am I best just to send the form off and wait and see?

I would send the form off and wait and see. If it's a problem, they will ring you and you can submit further choices by phone. So have a lengthy list to hand, as it can take many many choices before you get one that is acceptable.
