We used a fab site recommended on this site as approx 18mths ago. They could engrave on  both sides so we could put the fact that the pets were microchipped.
Of course we have lost any addresses/websites from them so would love to hear any recommendations.

Local vets (large ones) do an ID tag service, they send them away.  Mine is a large brass one with all the relevant info on including ID chipped and vets tele number.  If you look in the phone book and choose a large vets they will be able to help.  I was really impressed with our ID tag, it was a fiver.

Most key cutting places do ID tags while you wait.
