Can anyone point me in the right direction for a sample Stud Contract?

I/WE ____________________________________________ THE OWNERS OF THE BITCH

_________________________________ AGREE THAT:

  The bitch is / shall be in good health and free from infection at the time of service

  To the best of my/our present knowledge the bitch does not suffer from and is not a carrier for any inheritable disease

Should I/we become aware that the bitch suffers from or is a carrier for an inheritable condition at a later date I/we shall immediately inform **************  the owner/handlers of the dog 

  The bitch is / shall be between 2 years and 7 years of age at the time of service

  The bitch is / shall be presented at an appropriate time for service

  I/we shall supply an accurate 5 generation pedigree of the bitch at or before the service

  I/we shall supply a copy of the KC/BVA hip test and eye certificate (including glaucoma) which relates only to this bitch .

 
 
  To the best of my/our knowledge the bitch has not been  / will not be previously mated by any other dog during the season in which she is mated to the dog *************

  After the mating I/we shall ensure that the bitch is properly confined for the remained of her season so that no further accidental mating can occur

  I/we shall pay the fee of £ **  at the time of service ,(or £** if the bitch comes without an ovulation test)  plus £*** if the bitch is scanned in whelp at around 24 - 28 days post mating., or if the bitch subsequently whelps even if the scan is negative!

   
 
   
 
  Under no circumstances whatsoever shall I/we knowingly sell or transfer puppies whelped as a result of this mating to any person or firm connected with the business of resale or wholesale of dogs

I/we shall undertake to ensure that all puppies born of this mating are placed in suitable homes and are adequately followed up

  We shall take good and adequate care of the bitch should she remain in our charge

  We shall ensure that should the bitch remain in our charge she will only be mated by the dog ******** unless other instructions are given by the bitch owner

  We shall supply an accurate 5 generation pedigree of the dog at or before the service

  We shall supply a copy of the KC/BVA Hip score and eye test (including Glaucoma) relating only to this dog .

 
 
  We shall provide the bitch owner(s) with a written receipt for the stud service and a record of the breeding dates at the time of service / within ____ days of service

  We shall sign litter registration promptly and return it to bitch owner(s) at the time of service / within ____ days of service
