Help!!!!!
im having a nightmare im importing a dog and she has finished the precudure but i think im missing a form either the Pets form or the actual passport ,i have the  international vaccination book (yellow book)showing microchip number and certificate ,i also have the blood test result.and six months have elapsed from the test date but im sure i dont have the actual Pets form. ive contacted the issuing vet and he says i have evrything i need but defra say i must have either the new passport or the pets1 form, does anyone have a picture of the front cover of either of these so i can see if ive got it!!

Our new pet passport is a blue book that says PET PASSPORT in big letters and also says European Union, United Kingdom. It has the stars logo and a number.  Have e mailed pic if it works.

PS Pets 1 form says that at the bottom of the form and is an A4 size

i couldnt open the picture but im pretty sure i havnt got either of them!! i definatly havnt got a blue book,i now need a crash course in german to ask thew vet for the form again!!!
