Does anyone know if cataracts can be treated homeopathically or is surgery the only answer?
A friend of mine has a 6 year old spaniel which has cataracts in both eyes

No. The only treatment for a cataract, as far as cure is concerned is an operation. If it is not intended to operate on the dog then no treatment in required for the dog to live it's life out.

A slightly longer answer is, that if the cataracts are hereditory then the dog is unlikley to loose all it's sight because a hereditory cataract normally only coveres a small part of the eye.
