You've just told yourself what to take. :D
We go quite often on the ferry between Italy and Greece, and we'll be going on the ferry again in about 13 days with 3 dogs. :D You should take a crate anyway, and leave the puppy in the crate in the car. A crate also has the added advantage that if it's hot you can put towels on the windows and leave the doors open for ventilation without worring that puppy will escape.
If you can stay in your car then take this:
1 crate, plexiglass is good for safety and is more secure, but you might want to take along a metal collapsable one anyway.
4 towels, you never know when you'll need one. They are useful for sunscreens, mopping up puppy vomit, and many other things.
2 dog bowls, metal is best, very hard wearing plastic if you can't get metal. Dog bowls are useful for more than holding water and food.
About 5 liters of your water, strange water can give dogs the runs, also it is helpful to have some water on hand if the dog throws up or pees and you need to clean it up.
2 collars (or harnesses, depends on what you use) and 2 leashes, in case one breaks or is lost in the car. We usually have about 8 hanging around though. :D
Enough food for 7 days
1 or 2 rugs, for the dog to lay on
A muzzle, you won't need this now, but if it's a large breed you should take one along when he's an adult. We are sometimes asked to put Penny on a muzzle.
