Has anyone else received a questionnaire from the Kennel Club/British Small Animals Veterinary Association Scientific Committee/Animal Health Trust? There are questions on the health of your dogs,diseases,breeding,causes of death in your dogs and birth defects in your puppies to list a few. The questionnaire is designed to be competed for upto 10 dogs of the same breed. If anyone else is interested in receiving a copy, they are advised to contact their breed club or if you have received a copy, you are allowed to photocopy it and pass it on. Only one questionnaire per breed. All breed clubs will receive detailed feedback when all the information has been collated and it is hoped that the breed club and breeders will then act on the information provided. The information collated will also provide baseline information against which the success of future control schemes can be measured.

It is extremely important that as many people carry out this survey as possible.

Closing date: Friday 30th April 2004. Extended to 1st June.

The deadline for returning questionnaires was extended until the end of May.
We would be very grateful if you would mention this and encourage any late returners to complete their forms.  Every form returned adds to the value of the information that breeds will get back from the survey.  Information on healthy dogs is just as important as informationon diseases. If forms have been accidently lost or destroyed, duplicates can be obtained from the breed clubs that originally issued them or from the Health & Info Dept at the KC.

Yip got mine for the Breed club today in the post. Looks quite good I was really impressed.

It would really be nice to see health issues tackled as a concern in general and not breed selected. I know this is not 100% possible but a start would be good. Harmonising..:-)

Have sent mine off and can't remember which department was running the survey, perhaps some one can remember and let you know, sure a request would bring a form post haste.

Our breed club is issuing them & have to say that having read all the pages and leaflets, it makes my breed look very healthy now i have seen all the nasy diseases/problems available to dogdom! :)

does anyone have a copy? i have emailed my breed club twice and ive had no reply (not unusual im afraid) I would like to take part.:)

My breed club is sending a copy to all members in the next newsletter (due next month). Maybe I'll be able to take copies for people who haven't got one by then.

Got mine yesterday and posting it tomorrow -- well eldest is along with a bucket load of letters too.

I've had nothing!  Maybe because our breed is fairly rare they've not got in touch with us??  Not had anything from anybody re. Pom's either.  Has anybody got one that they could send me?

Still haven't got ours ..apparantly the KC sent them out and the breed club didnt get them ..so they were being sent again ..still not here though

Try the [email info@the-kennel-club.org.uk]Kennel Club[/email]. I would think that you will be able to get a copy at Crufts if you are attending.

A copy in PDF format is available on the KC website, under Health ,it is only a sample, unless you are a member of your breeds largest breed club it appears you are not eligible to complete this survey. However it does state that results will be sent to all Breed Clubs. I would have thought the more people they could get to fill in the survey the better.

Wonder why the breed clubs are taking such different lenghts of time to get these out. I got one from the breed club of my main breed weeks ago, but haven't got anything yet from the other two clubs I belong to.

The covering letter clearly states that you are welcome to photocopy your own copy and pass it to anyone with a pedigree dog who would be interested in taking part. I think this is important to do. There are many 'pet' owners who have pure bred dogs that are not members of any breed clubs. They still have problems with their dogs and I think it is important that their information is logged too.
