hello we have a litter of 8 black lab puppies and are all very healthy but one puppy(nelly) has a sort of nappy rash looking thing, and is very smelly and bit wet underneath, but when she first got the rash i dont think she was wet and smelly, have seen our vet and she said it could be urinary incontinence and would possibly have to be put down or have the operation (which would cost around £2000) which we cant afford, and i just keep thinking it is only a nappy rash thing??? she has  given nelly some anti-biotics and we hope it will clear up but please could you reassure me she will be ok?... as our friend who is having nelly is coming next sunday to pick her up,she has been waiting a long time for our doggies puppies (4 years) as she thinks our bessie is lovely but anyway she might not be able to even have her will she?? would be grateful for any advise??or otherwise??thank you

What operation did the vet suggest that would cost £2000?  When did Nelly start her antibiotics?

I don't think I would like to send a pup that wasn't healthy to a new home.  It would be best to keep her until everything is sorted and she is feeling on top form.  How old are the pups now?

I am more knowledgable people will be along soon to give more advice, but for now good luck.

It sounds to me from the symptoms & what your vet has suggested very much like she has an ectopic ureter. If that's the case - unfortunately she will need surgery. Which would be a very expensive operation or 2 (depending if it's a bilateral ectopic ureter).
I've seen pups with this that have been operated on & sometimes they are completely dry afterwards with no problems at all. Others will have slight incontinence which can usually be controlled with propalin drops or similar.
In the short term to make her more comfortable try & get some hibiscrub from your vet or a chemist & clean her vulva & surrounding area several times a day. Then put on some sudocrem to soothe if it's sore.
