Could those of you who have a form or letter that you get people to sign when they return a pup or adult please let me have a copy asap - ie this morning?

I do have one but would like to pick other people's brains :) so that I make sure every thing is covered

I don't have a standard letter/form, I write it out to suit what is happening etc.
The one I used yesterday for a family who didn't realise the commitment a pup needs just states:
We(name)............................................. of(address)..............................................relinquish all claim on the black labrador bitch (KC name...............................
Reg No.....................................
All rights are passed to (Breeders name).................................. and the dog will be kept at (Affix) or rehomed as she sees fit.

Not brilliant but it just covers anyone turning around and changing their mind the following week.

I have got what's called a surrender form if an owner brings his dog to my rescue:

I, ..........., hereby surrender my dog (name, breed, age etc) to ***** as founder and representative of ****. I know that with the signing of this form I forfeit any right to the above mentioned dog. I also know that the above mentioned dog will be homed as ********* sees fit.

Name, date, address etc
