Steve Mann (founder of the Institute of Modern Dog Trainers) has a great book called 'Easy Peasy Puppy Squeezy'. The book is presented as a step-by-step guide so it is really useful and easy to read.

I have read the book and would recommend it because it has detailed info (with pictures) on everything puppy related from loose lead walking, eye contact, socialising, crate training and reflex to name training. Steve Mann also provides a diary/notebook that you can use alongside his book. Check out his Amazon shop:
