how old does a puppy have to be to import from poland to uk?

A dog has to have all vaccinations with the latter one being Rabies. This cannot be done until the Pup is 12 weeks old, following the vaccination you have to wait a further 21 days before they are allowed in. All dogs be-it a pup or adult will have to have all vaccinations and wait for the 21 days following the Rabies Jab.

3 calendar months + 21 days - so just under 4 months old.

Sorry, Chaumsong is absolutely correct with 3 months then the 21 Days. I'm still working in "old money" where 12 weeks was also known as 3 months.

No the rule was changed and it now IS 12 weeks + 21 days. https://www.gov.uk/take-pet-abroad/rabies-vaccination-boosters-and-blood-tests

7. Rabies vaccination, boosters and blood tests

You must get your dog, cat or ferret vaccinated against rabies before it can travel. Pets must be at least 12 weeks old when vaccinated against rabies for pet travel.

In my breed where a quarter of the UK population are imports we discuss this a lot. Although the government site does say twelve weeks, it also says the vaccine must be  "given according to the recommendation on the vaccine manufacturer’s data sheet"

Nobivac own site says "Primary vaccination age* 3 months or older"

It's splitting hairs but for the sake of a few days I'd wait till 3 months to be sure a particularly pedantic passport checker didn't pick up on it.
