So I want to change the name if my dog as well as ad a kennel name do I have to change the name then ad the addition of a kennel name or can I do both at the same time

I would assume you can do both at the same time -but you need different forms. Also remember you can only change the name if it has been no more than 3 months since the dog was registered, and the breeder (if not you) has to sign the form to agree to the name change.
