Been ages since I had to register my affix on a puppy, what is the form called, or can you do online???

The form is called *Application to change the Registered name by the Addition of a Kennel Name* not sure if you can do it online yet.

Its KC form 8 and can be downloaded from KC website as I have done it myself.
