Could be any number of things it would take a physical examination to know for sure so I would say you need to see the vet.

Yep, same here, you need a trip to the vet.

Can't see how worms would cause skin problems, unless your thinking of Ringworm which is a skin parasite and not a worm.  The Vets the best place to go.
