I have an 11 week old pup who cannot get out of the habit of urinating in the house. The pup doesn't leave large puddles, she will make small dots around the house (6 - 8 times per day). Any ideas??

You haven't said what training method you have used so far so I can only suggest you go right back to basics i.e. immediately on waking, after eating and every 45 mins playtime accompany puppy outside and stay with her until she wees lavishing praise as she does so.  I find it helps greatly to use a command word, as soon as puppy squats give the command followed by praise, after several days you will find you can get ahead of her with the command word which will cause her to speed up the whole business, if she doesn't respond to the word go back to step one for a few more days.  I never chastise for accidents and personally I would not let pup have full access to the house, keeping her to easy clean areas such as the kitchen until pretty much reliable.

6-8 times a day isn't a lot for a pup to wee, but if you're concerned it woulnd't be a bad idea to see a vet to rule out a urinary infection.  What method of training are you using at the moment.

I agree with Isabel back to basics but I would take puppy out into the garden more frequently than every 45 mins .
I have a 17 wk old Beardie puppy who has been extremely easy to house train, I kept her in the kitchen with me for most of the day letting her out approx every 20 minutes when she was playing to <be a good girl> I used training pads in the night which she used in the beginning and in the evening when we were in the lounge for little accidents.
You will notice by her body language when she wants to toilet < starts to circle and sniff the floor > if you are quick and get her attention you can quickly get her outside and remember lots n lots of praise .
