I don't know of any that you pay a whole lifetime's premiums up front

usually what happens is you pay a higher monthly or annual rate than for the other covers that a particular company offers.
Like I've said on a previous thread, M&S offer lifetime policy which is very reasonable and I think it's something like an extra 14p a month for me on each dog to cover the cost of not having any excess on claims either.
No doubt others will be able to give you some positive feedback on good companies and the best policies each offer but not sure about the upfront element - we'll see ;)
