Long shot, but has anybody got an unfilled out Idexx progesterone testing form they can scan and e mail to me. I am just sat here thinking about first blood test Day 10 Monday 0830 and suddenly realised that the testing kit I ordered on Monday last, hasnt arrived!  I have a spare phial for blood here but no form to fill in and post off with the blood. PLEASE if anyone can help I will be very, very grateful

In the past I have downloaded a form from the Idexx website. I used the Reference Laboratory Requisition Form for Small Animal. Tick the PGT box under Reproduction.

Thanks to all that PMed and replied. By luck I found someone who had a spare kit and after a few logistical  hurdles I managed to get it in time
