One of my pups now nearly 8 was diagnosed with this a couple of weeks ago after his nose became very sore and bleeding and the vet did a scope up it and took swabs.
A speciacilat vet has done an operation last week ( £1000 + ) on him and put in anti fungal treatment. If this doesn't work the advice was that he might need another op in 4 weeks time.
His owner has emailed this morning to say that he is in a poor state with pain and his nose really bleeding, also a high temp over the weekend despite being on antibiotics. He had a tummy upset due to the pain relief and now not eating.
The vet has now advised that it looks like the op hasn't worked and he will need another one in a few days (just over a week from the first op), she said the bleeding isn't a good sign. His poor owner is so upset to see her dog like this and is not sure whether to put him through another operation but is going to ask the vet for honesty regarding prognosis.

Has anyone had experience of this condition and recovery? I really want to give her some hope, her email was very sad.

Sounds awfull on the little bit of reading I have just done but I did find this you may find intertesting. Seems to depend on the type the dog has.

:- The prognosis is generally good for cases with localized nasal aspergillosis. The prognosis for systemic aspergillosis is guarded to poor, especially if there is evidence of sinus destruction.

It it something I suppose all our dogs are subject to if they have a supressed immune system. Tends to be triggered from mouldy leaves and my dogs are forever sniffing through mounds of leaves this time of year.

Yes does depend on the type but his is getting worse and I think just started in one nostril but is in both now, very very sore and painful.
I have asked her to check with vet about auto immune thing because I had read that too and I guess without establishing that you'll never going to win.
Mine too are always deep in leaves and and there have been so many more fungi around this year, not sure if that would make a difference though.

My first Dobermann had this. It was many years ago, early 90's, so my memory may not be 100%. :)

The first episode was treated with vit. k injections and antibiotics but when, a few weeks later, it came back she had an operation and was in the vet's for just under a week. From what I remember she had 2 rubber tubes running from the top of her skull, through holes drilled into the nostrils (it certainly didn't look very nice) which were flushed daily with some sort of anti-fungal solution. When she came home, having had the tubes removed there was a fair amount of swelling which did eventually subside, and she lived until almost 14 with no further episodes.

HTH and I wish him a speedy recovery. I'm sure treatments have changed in the 20+ years since Heidi had it.

I don't have any experience of this but on googling it I found this page

It makes reference to if the infection has spread it can cause not eating and fever (they class these as more serious signs). These things ring a few bells with what you posted. I would be asking the vet if the infection is likely to have spread because from what I have read on various online articles the prognosis once it has spread is not good.

I am worried for him and his prognosis. Haven't heard back from her since this morning so I am hoping he has started to eat again.
The op he had did include holes being drilled and anti fungal medications being adminstered, but it sounds like its got a lot worse. He didn't stay at the vets but like you say treatment has probably changed and the drains not required.

I am currently going through this awful disease with my dog too :( He had the sinuses drilled and antifungal drugs infused into his sinuses and nasal cavity 2 weeks ago. Initially he did well, but is now off colour, and off food too. Hopefully this is due to a secondary infection, but if there is not a quick improvement on antibiotics, he will have to go back in for more surgery...unfortunately this still involves the week long stay at the vets with indwelling catheters placed and the nasal cavity and sinuses flushed out twice daily. I am dreading having to make the decision to put my dog through this too. Sympathies with everyone else who has to deal with this horrible disease. The success rate of treatment with the nasal form is supposed to be about 60 to 80%, but it sounds to me that multiple treatments are often required. Best of luck to your friend.

Hi, I work with aspergillosis in humans and unfortunately, when systemic infection is involved, the prognosis is grim there too. We are only now discovering more effective ways of treating this invasive and speedily progressing disease.

It's important to find out what kind of aspergillosis the vet is referring to: chronic pulmonary, invasive pulmonary, aspergilloma? The former needs long-term steroids and antifungals, but it is not curable! Only manageable -- at least in humans.

Aspergilloma can often be resected by surgery; advise on giving a peritoneal lavage with an antifungal or general antimicrobial (e.g. taurolidine).

Invasive aspergillosis has a poor outcome; people and dogs are unlikely to survive. The only treatment is immediate high-dose antifungal therapy, alongside antifungal susceptibility testing (expensive!) to check that the specific type of Aspergillus causing the infection can be killed by the drug used.

Hope this helps.
